Der problematischste Teil der Gallenblase ist das Infundibulum, weil es dort eng wird und der Stein Wand an Wand liegt und damit keine Fl\u00fcssigkeitumgebung, die f\u00fcr Impedanzunterschiede sorgt. Achten Sie daher auf indirekte Steinzeichen: einen hellen Oberfl\u00e4chenreflex und v.a. eine Schallausl\u00f6schung. Koliken seit Tagen, dann perforiert die Gallenblase und der Inhalt mit Eiter, Sludge und Steinen abszediert in die Leber und sorgt f\u00fcr Schmerzerleichterung. Im Ultraschall sehen Sie im Rippenbogenrandschnitt oder auch transcostal rechts eine un\u00fcbersichtliche Situation. Er steht vor mir, der Bauch ist sehr gro\u00df und er deutet auf ein ausgepr\u00e4gtes Caput medusae externum. Alkoholkonsum wird verneint. Ich habe Ihnen rechts unten einen korrespondierenden Normalbefund eingeblendet (mit ein paar Leberzysten). Zuerst kommt der Abdomenschallkopf, dann der Linearschallkopf. In diesem typischen Oberbauchquerschnitt bzw. Rippenbogenrandschnitt versuchen Sie zuerst die Lebergef\u00e4\u00dfe auseinanderzuhalten. Was sind Venen, was Portalvenen, was Galleng\u00e4nge. Was ist hier ver\u00e4ndert? Vergleichen Sie immer mit dem Normalbefund rechts unten. Zuerst kommt der Abdomenschallkopf, dann der Linearschallkopf. Welche Gef\u00e4\u00dfe zeigen diese kr\u00e4ftigen echoreichen bandf\u00f6rmigen Einscheidungen?<br \/>\r\nRichtig, es sind die Portalvenen. Es ist ein absoluter Klassiker, den Sie hier sehen.<br \/>\r\nWeltweit die h\u00e4ufigste Ursache einer portalen Hypertension sind nicht der Alkoholkonsum oder die Virushepatitiden. Wenn man alle Regionen der Welt, die entwickelten und die weniger entwickelten ber\u00fccksichtigt, dann ist es der P\u00e4rchenegel, also die Schistosomiasis. Es gibt die Unterarten Schistosoma mansoni, japonicum und auch guineensis. Vermutlich ist es letzterer bei unserem Patienten, der aus Guinea kommt. Wo und wie man sich die Schistosomen einf\u00e4ngt, sehen Sie in der Grafik.<br \/>\r\nWichtig ist, dass Sie die Einscheidungen der Portalvenen, die \"Symmer\u00b4schen Pfeifenstielfibrosen\" erkennen. Es ist eine Blickdiagnose im Ultraschall. Soweit, so klar. M\u00f6glicherweise ein Lemmel-Syndrom (divertikelbedingte Abflussbehinderung mit Steinbildung): ERC, Papillotomie und Steinentfernung, ein Jahr sp\u00e4ter Gangrevision mit Entfernung einzelner Steine. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Vor einigen Monaten kommt sie wieder. Im Gallengang ist Luft und etwas Sludge, keine Steine, kein Stau.<br \/>\r\nJetzt liegt das Endoskop im Magen und wir sehen in den rechten und hier in den linken Leberlappen: <br \/>\r\nDie linkshepatischen Galleng\u00e4nge sind z.T. tubul\u00e4r aufgeweitet und Sie sehen mehrere kleine Steine mit Schallschatten, die sich bewegen. Das fehlende Signal im Farbduplex best\u00e4tigt, dass es Galleng\u00e4nge sind. Au\u00dferdem findet man eine periduktale Fibrose. Ihre Diagnose?<br \/>\r\nRichtig, eine linkshepatische Caroli-Krankheit. Ich habe mit dem Schallkopf palpiert und gesch\u00fcttelt, um zu sehen, ob Steine oder Sludge aufschwimmen. Unsere Patientin hat bei der Schallkopfpalpation keine Schmerzen.<br \/>\r\nDie Gallenblase zeigt besonders im schallkopfnahen Fundus ein massives zwiebelschalenartiges Wand\u00f6dem und ein komprimiertes Lumen ohne Steine oder Sludge. Die Kinderchirurgin fragt, ob es nicht besser sei, diese Gallenblase zu operieren. Was antworten Sie?<br \/>\r\nDer CEUS zeigt eine anatomisch intakte Gallenblase mit einem ringf\u00f6rmigen Einstrom der Bl\u00e4schen im normalen Gef\u00e4\u00dfbett der Gallenblase. Kein Absze\u00dfbild. Die Milz ist stark vergr\u00f6\u00dfert (Foto s.u.). Zusammen mit der Lymphozytose mit Reizformen erh\u00e4rtet das Labor Ihren Verdacht einer infekti\u00f6sen Mononukleose. Damit handelt es sich um eine Mitbeteiligung der Gallenblase bei einem hepatotropen Virusinfekt, so wie bei den Hepatitiden, EBV, HSV usw.
